Please review the following information carefully. Thank you. 1). CCT is not responsible for damages to items that have been self-packed by owner. 2). Valuation: Sixty cents per pound per article unless specifically excepted (see Notice). 3). Notice: The shipper, subject to and based on the rates, rules, regulations, and conditions in the carriers lawfully published tariff hereby orders the carrier to furnish transportation facilities and service described subject to all conditions. Unless credit arrangements are made in writing the Shipper agrees to pay charges in cash, money order, or certified check prior to complete delivery. 4). Carrier Liability: Shipper declares full value of the shipment for the purpose of carrier liability. 5). Delivery Receipt: Customer- except as specifically endorsed, are expected to sign a receipt indicating all services and all articles are received in good condition. 6). Finance Charge: After 30 days- 1% monthly; and 12% annually. |
